 Patient Care and Family Support

Hospice of the Foothills’ interdisciplinary team uses a comprehensive
patient and family-centered care approach that is developed
to uniquely fulfill each patient’s care needs.

A typical care plan includes pain and symptom management, personal care, social services, spiritual support, respite care, grief support and bereavement services. Our professional care team consults with the patient, his/her physician and family to develop a specialized plan of care with clear patient-centered goals of care. Our professional care team meets every two weeks to discuss the progress of each patient according to their plan of care. This attention to detail and our multi-disciplinary approach offers a high quality experience grounded in excellence and compassion.

The core concepts of Patient and Family-Centered Care are:

  • Dignity and Respect -To Listen to and honor patient and family ideas and choices and to use patient and family knowledge, values, beliefs and cultural backgrounds to improve care planning and delivery.

  • Information Sharing - To communicate and share complete and unbiased information with patients and families in useful ways. Patients and families receive timely, complete and accurate details so they can take part in care and decision making.

  • Involvement -To encourage and support patients and families in care and decision making at the level they choose.

  • Collaboration -To invite patients and family members to work together with health care professionals to assess and improve programs and services.

Controlling pain and symptoms

Our Medical Director and nursing staff work closely with patients, their families and their attending physicians to control pain and alleviate any other symptoms. Hospice of the Foothills provides prescription medications necessary for symptom management and pain relief as they relate to the hospice diagnosis, at no cost to the patient. Clinical services and durable medical equipment may also be part of the care plan.

Spiritual support

Spiritual counselors support patients and family members with any spiritual issues that might arise during this difficult time such as the meaning of one’s life. Performing a life review is one way to accomplish a sense of peace for the patient as well as the family.

Personal care

Hospice aides offer assistance with bathing and personal care for patients throughout their time in hospice care.

Social services

Social workers provide guidance and help with end of life tasks, financial issues, and final arrangements. It can be difficult to keep track of all of the details involved in end-of-life care and our team of trained professionals wants you to have some peace of mind that your family is also being looked after.

Respite

We understand that caregiving often means round-the-clock care. We know it’s challenging to offer the best care possible to your loved one even in the best of times. Hospice of the Foothills offers short-term respite care in a facility where professionally trained caregivers take care of your loved one to give you a break

End-of-Life Option Act

The end of Life Option Act provides aid in dying for qualifying terminally ill adult California residents who have an anticipated life span of 6  months or less. The patient must possess intact decision making capacity and be able to self-administer the oral medications. Hospice of the Foothills neither advocates for, nor opposes Physician Assisted Death.
