History

Hospice of the Foothills began as an all-volunteer operation, providing services to communities in Nevada and Placer counties. Since its founding in 1979, Hospice of the Foothills has evolved from a small, all-volunteer operation, to the professional, interdisciplinary care team of today. In addition to the programs and services provided, continues to focus on outreach, collaboration, education, and forming partnerships with others in our community. Hospice of the Foothills is a non-profit 501(c)(3), serving over 500 patients and families in Western Nevada County and surrounding communities every year.

Mission

Our mission is to provide compassionate end-of-life support for patients, families and the community.

We recognize and celebrate the rich tapestry of diversity that exists within our organization and the communities we serve. We are dedicated to creating an inclusive environment that values and respects differences in age, race, ethnicity, gender, sexual orientation, religion, physical ability, socioeconomic status, and beyond. We understand that diversity enriches our perspectives and enhances our ability to provide compassionate and competent care.

Philosophy

Patients and their families are at the center of the care we provide. The philosophy of hospice is to recognize that quality of life, peace, and comfort at the end of life should be the focus when curing a patient’s disease is no longer possible (National Hospice and Palliative Care Organization). HOF team members accomplish this through an interdisciplinary plan of care developed with the patient and their loved ones that enhances the patient’s quality of life by:

  • Offering symptom and pain control

  • Promoting comfort

  • Exploring spiritual connection

  • Providing psychological support

  • Establishing connections for the support of daily life

  • Continuing support for loved ones throughout the bereavement period

We assist dying patients and their families to attain a quality of life which allows them to live life to the fullest and to prepare them mentally and spiritually for their end of life journey.

We believe that patients have the right to be informed about their disease, prognosis, treatment, and available alternatives. Patients and families are the primary participants in the decision-making process and have the right to accept or refuse treatment.

HOF subscribes to a holistic approach concerning the physical, psycho-social, and spiritual needs of our patients. Hospice support is available 24-hours a day, 7-days a week. Bereavement follow-up and grief support is considered part of the total care concept and is available to the entire community.

We are committed to the promotion of comfort and to the relief of pain and other distressing symptoms. We promote an environment of mutual trust, respect, support, and skill to enable the patient to receive care where ever they reside.

Hospice of the Foothills is licensed as a hospice by the state of California and certified by the Centers for Medicare and Medicaid Services (CMS).
