Triage Nurse – Full Time, Benefited

Summary of Duties and Responsibilities: As member of the patient care support team, the Triage Nurse is responsible for providing information and consultation regarding hospice services to patients and families.

Primary Department: Patient Care Unit

Reporting Relationship: The Clinical Care Manager

Position Type: Non-Exempt, Full Time, Benefited position, AWS – 10 hour shifts

Job Details and Essential Functions:

  • Responds to patient and family calls by making decisions utilizing independent judgment and works effectively in atypical, urgent and crisis situations
  • Provides triage for home visits, hospitalization, or physician office visits and nursing advice to determine immediate level of care required
  • Attends weekend death events if no RN is available
  • Responsible for notification to attending physician, DME, pharmacy and any other entity of a Hospice of the Foothills (HOF) patient’s death
  • Assists with managing/processing of faxes/orders as assigned
  • Approves and processes orders from the physician’s portal daily
  • Supports Intake nurse by responding to phones calls, processing referrals, determining, with hospice Medical Director, eligibility and initiating admission process
  • Participates in Quality Assurance Performance Improvement as assigned
  • Is knowledgeable of agency guidelines regarding patient referrals, physician/discharge planner relationships, and consumer complaints
  • Serves as a field nurse as needed during staffing shortages and vacations
  • Shifts include rotating weekends
  • Performs other duties as assigned

Knowledge and Skills

Required

  • Ability to use independent judgment to make sound decisions
  • Ability to maintain confidentiality and follow HIPAA and patient privacy laws
  • Strong, clerical skills including typing, copying, scanning and faxing
  • Ability to critically think and provide necessary interventions
  • Ability to communicate effectively, both orally and in writing

Education & Experience

Required

  • Minimum three-years hospice, home health or community health experience within past 5 years
  • Excellent computer skills and understanding of computer environment and terminology
  • Demonstrated communication and interpersonal and nursing skills, adaptability, organization, thoroughness, responsible and leadership qualities

 Desired

  • Knowledge of non-cancer diagnosis criteria State and Medicare hospice regulations desirable
  • Hospice experience

 License / Certifications

Required

  • Maintain and possess current RN License in the state of California
  • Possess and maintain valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance
  • Possess and maintain current CPR certificate

Desired

Hospice and Palliative Nursing Association certification
