What is Palliative Care?

Palliative care is specialized medical care for people with life-limiting illness. Care plans focus on providing relief from related symptoms, pain and stress.  Palliative care can be provided at any stage of a serious illness, even while pursuing curative care.

When you are admitted into the Foothills Palliative Care Consultants program for home-based palliative care, our experienced team of professionals become your healthcare advocates. We aim to reduce unwanted hospitalizations, make patient care easier, and provide a better quality of life.  Additional benefits include:

  • Partnership with the patient’s attending physician for all health care decisions
  • Personalized medical care plan supervised by a nurse practitioner case manager
  • Knowledge and expertise provided by a licensed clinical social worker
  • Proactive pain management
  • Referrals to a large array of community programs such as Meals on Wheels, support groups, housing grants, veterans support groups, and paratransit services
  • Access to case management 24 hours a day, seven days a week

When is the right time to call the Foothills Palliative Care Consultants?

Call for help if someone you know is dealing with a life-limiting illness and:

  • needs help obtaining information and understanding the illness and its challenges
  • is having trouble managing the activities of daily life

Who pays for the Foothills Palliative Care Consultants program?

Program services will be billed through the patients’ insurance. The patient may be responsible for co-pays.

Who qualifies for the Foothills Palliative Care Consultants program?

Criterion for admission into the program is the diagnosis of a life-limiting illness with prognosis of one year or less. Referrals for the program can be made by a community based physician (including hospitalists), a patient or family member, or by Hospice of the Foothills’ Medical Director.  If you have questions about eligibility or obtaining a referral, please call 530.272.5011.
