Position Summary:

Greets customers and receives donation of goods, presorts donations, assists customers in unloading/loading heavy items, drives Hospice Of The Foothills vehicle to pick up donations. This is a non-exempt position.

Warehouse Receiver Part Time

Admission’s RN

Accountable to:

Intake and Admission’s Supervisor

Position Summary:

This is a full time position; 32 – 40 hours per week covering Monday through Friday and rotating Saturdays. A Registered Nurse License is required; performs consultation and admission visit to assess patient’s needs and completes admission assessment and contractual agreement documentation for Hospice of the Foothills program. This is a non-exempt position.
Possesses understanding of the Hospice program and benefits.
Obtains orders for and procures immediate medications and needed DME/supplies.
Performs physical assessment of patients’ condition, safety, knowledge, pain and symptoms.
Initiates the Plan of Care,
 Instructs on disease understanding, progression, and immediate interventions.
Documents assessment, current medications and initiates appropriate care plans.

Education & Experience
1. Possess and maintain current California RN license.
2. Possess and maintain current CPR certificate.
3. Possess and maintain valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance.
4. Knowledge of State and federal Medicare Hospice regulations desirable.
5. Knowledge of terminal care criteria and end of life issues.
6. Demonstrate communications and interpersonal skill.
Hospice of the Foothills
7. Demonstrate adaptability, organization, attention-to-detail, thoroughness, responsibility and leadership qualities.
8. Minimum one-year RN experience, preferable in Hospice, home health, community health or Medical/surgical area.
9. BSN with public health certificate preferred

Nurse Practitioner (NP)

Accountable to:

Director of Patient Care Services/CCH Facility Administrator; Clinical evaluation by physician c0-practitioner.

Position Summary:

General:  The NP provides/enhances patient care based on advanced nursing practice principles in area of specialty. Provides on-call services and patient visits as governed by protocols approved by the supervising physicians with appropriate physician consultation. Clinical Practice: The clinical practice is based upon the job description, approved scope of practice by the BRN and Standardized Procedure.

Visit Nurse – Per Diem (VN-PD)

Accountable to:

Nursing Supervisor (NS)

Position Summary:

The VN-PD reports directly to the NS.  Provides skilled nursing care as needed to supplement staffing and to assist the RN Case Manager. This is a non-exempt non-benefited position.

Medical Social Worker – Per Diem (MSW-PD)

Accountable to:

Medical Social Services Supervisor (MSSS)

Position Summary:

Reports directly to the Medical Social Services Supervisor. Responsible for the delivery of social work services including initial psychosocial assessments, ongoing counseling, direct casework services, bereavement services, community education, outreach and referral. This is a per diem non-benefited position.
