Certified Home Health Aide (CHHA)

Summary of Duties and Responsibilities: Under the supervision of a Registered Nurse (RN), the CHHA provides basic nursing aide services to ensure the safety, comfort, personal hygiene, and protection of our patients. The CHHA will assist patients to participate in planned activities designed to make life more meaningful, to stimulate and support physical and mental capabilities to the fullest extent and enable the patient to maintain social, physical and emotional functioning appropriate to their level of care.
Primary Department: Patient Care Unit (PCU)
Reporting Relationship: Director of Patient Care Services (DPCS)
Position Type: Non-Exempt, Full Time, Benefited
Salary Grade:     Salary Range $14.70 to $20.35

Job Details and Essential Functions:
• Adheres to written and oral instructions for patient care as prepared by the RN
• Assists the patient with all aspects of personal care, ambulation, positioning and feeding as required
• Provides support to the family/caregiver regarding personal care
• Performs light housekeeping services that are necessary for the health and safety of the patient/family/caregiver
• Follows the guidelines for Standard Precautions at all times
• Maintains records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by Hospice of the Foothills (HOF)
• Maintains strict confidentiality of patient/family/caregiver information
• Attends and participates in team meetings, morning report, in-services, department and agency meetings and other related activities
• Adheres to all policies, procedures and regulations regarding patient care, conduct safety, fire, security and risk management
• Complies with mandatory abuse reporting requirements
• Understands and adheres to the organization’s mission, policies and procedures and advocates its philosophy.
• Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service representing the organization to all customers with integrity and professionalism
• HOF may assign the CHHA to any workplace setting based on HOF needs including office, RCFE, SNF and the patient’s home
• Other duties as assigned, including work in other areas to cover absences or relief to equalize peak work periods or otherwise meet the needs of the organization

Knowledge and Skills
• Demonstrated proficiency in Hospice care delivery techniques competencies and procedures
• Dedication to record keeping requirements
• Routine CHHA skills practiced in a healthcare environment
• Intolerant of conditions that are symptomatic of neglect or abuse
• Uses independent judgment and decision-making to organize work and establish priorities
• Must be able to relate well to death/dying situations and support the Hospice concepts
• Must have excellent organizational skills and ability for multi-tasking Must be able to complete required paperwork with accuracy and meet prescribed deadlines
• Must be able to work without continuous supervision
• Sensitivity to and ability to interact with various racial, religious, and ethnic cultures in the organization service area
• Ability to solicit sensitive information from patients and their families/caregivers
• Ability to remain calm with patients and their families/caregivers under fast paced, emotional conditions

• Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation.

Education & Experience
• High School diploma or equivalent
• Meets competency criteria as established by State and Federal requirements & Hospice of the Foothills (Required experience must be in the recent past- not to exceed 24 months)
• Demonstrated communications and interpersonal skills, adaptability, organization, attention-to-details, thoroughness, responsibility and able to follow directions
• Expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities, and to attend mandatory educational programs
• 1 year experience as CHHA or CNA in a clinical setting
• Knowledge of death and dying process

License / Certifications
• Valid CA Drivers License and a reliable vehicle for transportation to accomplish work.
• Possess and maintain current Home Health Aide (HHA) Certificate.
• Possess and maintain current a CPR certificate.
• Certified Nursing Assistant (CNA)
