Summary of Duties and Responsibilities:    Performs consultation and admission visit to assess patient’s needs and completes admission assessment and contractual agreement documentation for Hospice of the Foothills program.

Reporting Relationship: Clinical Care Manager

Job Details and Essential Functions:

Admissions Nurse is responsible for assessing our patient’s medical status, planning, teaching and implementing immediate medical interventions on the day of admission.

  • Responsible for complete patient admission process. Obtains orders for and procures immediate medications and needed DME/supplies. Performs physical assessment of patients’ condition, safety, knowledge, pain and symptoms. Initiates the Plan of Care. Instructs on disease understanding, progression, and immediate interventions. Documents assessment, current medications and initiates appropriate care plans
  • Assessing a referred patient’s medical status, planning, teaching and implementing immediate medical interventions on the day of admission
  • Possess understanding of the Hospice program and benefits
  • Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
  • Participate / contributes in ongoing interdisciplinary comprehensive assessment
  • Develop and evaluate the Plan of Care (POC)
  • Contribute to patient and family counseling and education
  • Coordinate medications and DME needs for same-day delivery
  • Collaborate with attending physician, Hospice Medical Director, Clinical Coordinator and Social Worker regarding patient admission and follow up needs
  • Responsible for completion of all elements of admission process and first 24-hours care
  • Perform timely administrative functions relating to admissions, clinical records, Plan of Care, mandatory abuse reporting requirements, and Home Health Aide assignments
  • Communicate admissions information, issues and concerns in patient care meetings to assure smooth transition and continuity of care with other assigned staff, including a detailed report to the assigned Case Manager and assigned Social Worker
  • Provide consultation and emotional/physical support to patients/families regarding Hospice services and disease processes
  • Participate in community education, volunteer and Hospice activities as requested
  • Serve as backup to Intake Coordinator, RNs and Triage as requested
  • Performs routine nursing visits when required
  • Participates in QAPI program and Hospice sponsored in-service
  • Performs other duties as assigned that are consistent with meeting departmental and organizational goals and objectives
  • May be assigned to assist in other work areas such as triage, intake, SNF/RCFE, or other areas assigned by DPCS or Clinical Care Manager.

Knowledge and Skills

Required

  • Ability to use independent judgment to make sound decisions
  • Maintain confidentiality and follow HIPAA and patient privacy laws
  • Strong clerical skills including computer, typing, copying, scanning and faxing.
  • Critical thinking and provide necessary interventions
  • Ability to communicate effectively, both orally and in writing
  • Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation
  • Meet deadlines and remain on timelines

Education & Experience

Required

  • One  year experience as an RN in Hospice, home health, community health, or oncology unit or two years experience as an RN in a Medical/surgical area
  • Participate in appropriate continuing education
  • Expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities, and to attend mandatory educational programs

Desired

  • Three years experience as an Admissions RN for a Hospice

License / Certifications

Required

  • Possess and maintain current California RN license
  • Possess and maintain current CPR certificate
  • Possess and maintain valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance

Desired

  • Certified Hospice and Palliative Nurse  (CHPN)
